Remove workarounds for Boost < 1.44

I'm proposing making Boost 1.44, which was released over 10 years ago, the minimum version required for graph-tool. The oldest version of Boost that Ubuntu still supports is 1.54, and the oldest version that Red Hat supports is 1.48, so everyone should be using a version of Boost newer than 1.44 by now anyway.
